Cefotaxime

Pharmacokinetics

Cefotaxime is administered intraveneously or intramuscularly.Cefatoxime is deacetylated in the body; the metabolite exerts weaker but synergistic action with the parent drug.Its half life is 1 hour, but is longer for the deacetylated metabolite hence permitting 12 hourly dose in many situations.

Indications

Cefotaxime exerts its action against most gram negative aerobic bacteria as well as some gram positive bacteria like Enterobacter, Klebsiella and Proteus and also against Beta lactamase producing bacteria. It is strongly indicated in gram negative bacillary meningitis, hospital acquired infections, and infections in immuno-compromised patients.

Routes of Administration and Dosage

Parentral dose: Adults: upto 12gms / day in divided doses at 6 or 8 or 12 hourly interval.In children 1 month to 12 years ; 50 mgs to 100 mgs/day in divided doses. . Newborns upto 1 week of age: 50 mg per kilogram (kg) of body weight every twelve hours, injected IV. Newborns 1 to 4 weeks of age: 50 mg per kg of body weight every eight hours, injected IV. Gonorrhoea is usually treated with a single dose of 250 milligrams (mg), injected IM.

Contra Indications

Contraindications are any previous hyper sensitivity to any of the cephalosporins /penicillins and severe renal impairement.

Precautions

Cross senstivity in penicillin hypersensitive patients has been reported.

Interactions

Activity may be enchanced by aminoglycosides.
