Pharmacokinetics Diclofenac is a phenylacetic acid derivative . It is a potent cyclo-oxygenase inhibitor. After oral administration it is well absorbed, and is 90% protein bound. It is metabolised and excreted both in urine and bile. It has a half life of about 1-2 hrs.It has good tissue penetrability and concentration in synovial fluid is maintained for three times longer period than in plasma, exerting extended therapeutic action in joints. Indications Diclofenac is indicated in painful inflammatory conditions associated with musculoskeletal system, dysmenorrhoea, during post operative and post traumatic period. It is given in chronic inflammatory conditions like Rheumatoid arthritis and osteoarthritis. Routes of Administration and Dosage Oral dose (tablet): For relieving dysmenorrhoea in Adults: 50 milligrams (mg) three times a day as needed. For rheumatoid arthritis in Adults : initially 50 mg three or four times a day, the dose increased up to a total of 225 mg a day. For delayed-release tablet dosage form: For rheumatoid arthritis in Adults: initially 50 mg three or four times a day, the dose increased up to a total of 225 mg a day. For osteoarthritis in Adults : initially 50 mg two or three times a day. Usually, no more than a total of 150 mg a day should be taken. For spondylitis in Adults : At first, 25 mg four or five times a day. For extended-release tablet dosage form: For rheumatoid arthritis, osteoarthritis, or spondylitis in Adults : Usually 75 or 100 mg once a day, in the morning or evening. For rectal suppositories: For rheumatoid arthritis, osteoarthritis, or spondylitis in Adults : One 50-mg or 100-mg suppository, inserted into the rectum. The suppository is normally used only at night by people who take tablets during the day. Usually, no more than a total of 150 mg of Diclofenac should be used a day totally. Contra Indications Diclofenac is contraindicated in hypersensitivity, active peptic ulcer, moderate to severe hepatic disease and in pregnancy. Precautions Those with Alcohol abuse, Colitis, Crohn's disease, diverticulitis, gastric ulcer, Diabetes mellitus, Hemorrhoids, Hepatitis, renal disease, Rectal irritation, bleeding, Systemic lupus erythematosus and Tobacco abuse may have increase chance of adverse effects. Interactions The chance of bleeding may be increased when Diclofenac is taken along with Cefamandole, Cefotetan, Heparin, Plicamycin, Valproic acid. When Diclofenac is taken along with Cyclosporine, Digitalis glycosides, Lithium, Methotrexate, Phenytoin there will be elevated serum levels of these drugs and an increased chance that adverse effects may occur.
