Pharmacokinetics Ergotamine is erratically absorbed from the GIT.Oral absorption is incomplete.It undergoes extensive first pass metabolism. Indications Ergotamine has alpha adrenergic blocking and vasoconstrictor properties . It is effective in migraine and tension headache. Routes of Administration and Dosage Oral and sublingual dosage forms- Adults: For relieving migraine; 1 mg is given at 30 minutes intervals till relief is obtained or a total of 6 mg is given.Dose of ergot should not exceed 10 mgs per week. For preventing cluster headaches : The dose of ergotamine, and the number of doses needed every day, will depend on the frequency of headaches/day. Usually 1 or 2 mg of ergotamine once a day may be enough, or if necessary 1 or 2 mg of ergotamine 2 or 3 times a day. For all uses: a total of 6 mg of ergotamine a day is the maximum permissible dose. For rectal suppository dosage forms- Adults: For relieving migraine or cluster headaches usually 1 mg of ergotamine, but the dose can range from 1/2 of this amount to up to 2 mg. If headache is not relieved, and there are no adverse effects, a second dose and even a third dose may be taken; however the doses should be taken at least 30 minutes apart. Adults should not use more than 4 mg of ergotamine a day in suppository form. This drug should not be used more than 2 times a week, at least five days apart. Children 6 years of age and older- For relieving migraine headaches: 1/2 mg or 1 mg of ergotamine. Children should not be administered more than 1 mg a day of ergotamine in suppository form. This drug should not be used more often than 2 times a week, at least five days apart. Contra Indications Ergotamine is contraindicated in hypertension, vascular and valvular heart disease, pregnancy, septic conditions, cachexia, collagen disorders and fibrotic disease. Precautions The chance of adverse effects may be increased in patients with Hypertension, pruritis, renal disease, hepatic disease, Mental depression, hyperthyroidism, intestinal obstruction and Urinary tract obstruction. Interactions When it is used along with Cocaine or parenteral Epinephrine adverse effects caused by ergotamine may be increased Brand Names Ergotamine combination brands: Migranil(Inga), Migril(Burroughs Wellcome), Vasograin(Cadila Pharma).
