Pharmacokinetics The loop agents are rapidly absorbed and handled by renal excretion . Duration of effect is for 3 to 4 hrs. Indications Loop diuretics are used to reduce oedema states . They work by inhibiting the reabsorbtion of Sodium and Chloride predominantly in the Loop of Henle. It is used to treat the oedema asscociated with congestive cardiac failure, hepatic and renal disease including nephrotic syndrome. Routes of Administration and Dosage For oral dosage form (tablets): Adults: 0.5 to 2 milligrams (mg) once a day. For injection dosage form: Adults : 0.5 to 1 mg injected IM or IV every three to six hours as needed. Contra Indications It is contraindicated in anuria, those with history of allergy to bumetanide, hypokalaemia, hyponatraemia, precoma stage associated with hepatic cirrhosis and addisons disease. Precautions Allergies :Allergic reactions to the drug may occur. Pregnancy: In general, diuretics are not useful for edema of feet and hands that occur during pregnancy. Diuretics should not be taken during pregnancy. Older adults: Dizziness, lightheadedness, or signs of hypokalemia may be more likely to occur in the elderly, who are more sensitive to the effects of loop diuretics. Elderly patients may also be more likely to develop thrombii. Interactions It may increase the nephro-toxicity of cephalosporins and aminoglycosides. Phenytoin and indomethacin decrease the effect of bumetanide. Analgesics may reduce the natriuretic effect. Bumetanide enhances the activity of muscle relaxants. Brand Names Bumet (Montari).
